Collateralization of Public Deposits
Collateralization Forms and Agreements You can view the Investment and Banking Forms below with the Adobe Acrobat Reader. Once you have the Reader, click on the form you wish to view. Now you can print the form, fill it out and mail it to the address on the form. A signature is required for each form.
Online Form Instructions To fill in a form online and print it in Acrobat Reader, follow these instructions:
- While you are viewing the form in Acrobat Reader, click on the "hand" button. This will enable you to click on a field and type.
- After the form is filled out, click the print button.
- All forms require a signature.
- Physically mail the form to the address located either at the top or bottom of each form.
Note: To enlarge the font size of a form while viewing it in the Reader, click on the magnifier button in the Acrobat Reader toolbar.

INV-91 Notification of Public DepositCompleted annually by the Public Depositor and submitted to the financial institution where funds are held, and provides a list of the accounts at the bank that should be included in Public Deposit calculations for collateralization purposes.

Note - All public funds on deposit with financial institutions must be insured against bank failure. The Collateralization of Public Deposits in NC document details how the financial institutions provide this insurance over and above what is provided by the FDIC to ensure that all public funds are made whole. |
